COVID-19 Procedure Agreement
I agree to quarantine for at least ten (10) consecutive days or receive a negative COVID-19 test before participating in the mission. Quarantine and testing requirements are attached. Attached self-screening must be completed each day of quarantine. Failure to do so may result in delay or allowance to participate. Participants who have immunity via past infection or vaccination within 90 days are exempt from these requirements, but must present proof of vaccination upon arrival.
Wearing masks for all indoor activities and during any outdoor activities when social distancing is not possible. Participants will not be required to wear masks and social distance from others in their “pod,” but pods will be required to social distance from each other.
Individuals who have received a COVID-19 vaccine or have previously been infected are exempt from the quarantine requirements below, but must still perform the daily self-screening. Please note the 90-day window of immunity the CDC identifies for both previously-infected individuals and those who receive the vaccine. If applicable, proof of vaccination must be presented upon arrival on the Opportunity. Do not copy or photograph the proof of vaccination.
OPTION ONE: 10-DAY QUARANTINE
Thank you for accepting the invitation to participate in Alaska: Anchorage (“Opportunity,” below.) To help protect yourself, Opportunity participants, and other members of the community, you must quarantine for ten (10) continuous days immediately before you begin traveling to your Opportunity location.
Related to the Opportunity, quarantine is defined as separating oneself from the community by restricting one’s movements and monitoring for any symptoms. Examples include staying at home, opening windows for fresh air, and remaining socially distant from the individuals that live with you.
Please use the COVID-19 Self-Screening Checklist daily to monitor your symptoms and ensure your readiness of participation. If you answer in the affirmative to any questions, please contact your Opportunity Leader. You only need to share that you have either become sick, developed COVID-19 symptoms, or have been exposed to COVID-19.
You must answer in the negative to all screening questions for ten (10) continuous days during quarantine, otherwise you may not be allowed to participate, or may be asked to delay your participation (in order to complete your continual passing of the daily screening.)
OPTION TWO: TESTING AND QUARANTINE
If you are unable to quarantine for ten (10) continuous days immediately before the start date of the Opportunity, you must take a COVID-19 test within 72 hours before the start date of the Opportunity and receive a negative test result. In addition, you must quarantine from the day you take the test until you begin traveling to your Opportunity location.
During this quarantine period, you should use the COVID-19 Self-Screening Checklist daily to ensure your readiness of participation. If you answer in the affirmative to any questions, please contact your Opportunity Leader. You only need to share that you have either become sick, developed COVID-19 symptoms, or have been exposed to COVID-19 and that either your test results are pending, or you tested negative.
You must affirm receipt of a negative test taken within 72 hours of the start of the Opportunity, affirm that you quarantined from the day of testing until traveling to the Opportunity location, and passed the COVID-19 Self-Screening Checklist in order to begin participating in the Opportunity.
If you receive a positive test result, then you may not be allowed to participate, or may be asked to delay your participation.
If you fail to completely follow either option, you may likely be prevented from participating in the Opportunity.
COVID-19 Self-Screening Checklist
To help protect yourself, Opportunity participants, and other members of the community, please review this checklist daily before engaging in the Opportunity’s activities. If you have any questions about this process, please contact your Opportunity Leader. Please use the provided thermometer to take your temperature each morning.
Are you sick?
- During pre-Opportunity quarantine: If yes, contact your healthcare provider, if necessary, and call your Opportunity Safety & Care Coordinator.
- During Opportunity: If yes, stay in your pod. Contact your healthcare provider, if necessary, and immediately inform your Opportunity Safety & Care Coordinator. Follow social distancing guidelines with your pod roommates and wear a face covering while in your pod, except when eating or sleeping.
Have you developed symptoms listed below?
- Have you developed any of the symptoms listed below that are different from your usual or baseline health? For those with chronic conditions, a positive screening should represent a change from their typical health status.
- During pre-Opportunity quarantine: If yes, contact your healthcare provider, if necessary, and call your Opportunity Safety & Care Coordinator.
- During Opportunity: If yes, stay in your pod. Contact your healthcare provider, if necessary, and immediately inform your Opportunity Safety & Care Coordinator. Follow social distancing guidelines with your pod roommates and wear a face covering while in your pod, except when eating or sleeping.
- Symptoms
- o Fever (100°F / 37.8°C or above)

- a) If an elevated temperature is the only symptom felt and you have answered “no” to the other two questions, then you may re-check your temperature after ten (10) minutes to see if it’s still elevated. If so, contact your Opportunity Safety & Care Coordinator.

o Chills (including chills with shaking)
o Cough
o Shortness of breath or difficulty breathing
o Fatigue
o Sore Throat
o Diarrhea or vomiting
o New onset of severe headache, especially with a fever
o New loss of taste or smell
Have you had close contact with someone diagnosed with COVID-19?
- During Pre-Opportunity quarantine: If yes, inform your Opportunity Safety & Care Coordinator.
- During Opportunity: If yes, stay in your pod and immediately inform your Opportunity Safety & Care Coordinator. Follow social distancing guidelines with your pod roommates and wear a face covering while in your pod, except when eating or sleeping.
If you answered NO to all the questions, you may participate in the Opportunity activities.
